해당 사업의 대상은 건강관리서비스 이용 및 접근이 어려우면서 건강관리가 필요한 지역사회 주민으로써 우선순위는 만65세 이상 노인, 기초생활수급자, 차상위 계층, 독거노인,만성질환자등 이다.
방문건강관리사업은 혈압, 혈당, 빈혈 등 기초검사를 포함해 만성질환 관리, 건강생활 실천을 위한 보건교육 및 상담을 제공하며, 필요 시 지역 내 보건·복지 자원과 연계해 지원한다.
아울러, 대상자 스스로 건강을 관리할 수 있도록 AI·IoT 기반 어르신 건강관리사업도 함께 운영하고 있다. 이를 통해 가정 방문이 없는 날에도 사물인터넷 기술을 활용해 건강 상태를 상시 모니터링할 수 있도록 지원하고 있다.
현재 양주시는 약 1,800명의 대상자 중 건강 고위험군 어르신 54명을 별도로 선정해 집중관리 서비스를 제공 중이다. 전담 방문간호사가 3개월간 8회 이상 가정 방문을 통해 만성질환을 집중 관리하며, 독거노인의 경우 우울감, 낙상 위험 등 지속적인 관리가 필요한 경우 AI 로봇을 추가로 설치해 건강 프로그램 전송, 말벗 기능, 투약 시간 알림, 응급 호출 등 다양한 서비스를 제공하고 있다.
