먹고 살기 힘들었던 과거에는 탈모가 있어도 별로 신경 쓰지 않았지만, 요즈음은 외모가 그 사람의 기준이 되는 시대이다. 따라서 탈모는 남녀노소를 불구하고 발생하면 스트레스로 작용한다. 그 이유는 두피에 머리털이 없으면 실제 나이보다 더 먹어 보이기 때문이다.
누구나 나이가 들어 보이는 것은 썩 기분 좋은 일은 아니다.
이런 탈모인들에게 구세주 같은 약물이 탄생했다. 바로 피나스테리드 성분인 프로페시아다. 프로페시아는 원래 전립선비대 치료제로 개발되었는데 탈모에 효과가 있어 탈모치료제로 출시된 것이다. 전립선비대 치료용으로 피나스테리드 5mg, 탈모치료용으로는 피나스테리드 1mg이다.
전립선비대와 탈모는 디하이드로테스토스테론(dihydrotestosterone: DHT)이라는 물질 때문에 발생한다. 따라서 DHT의 생성을 억제하면 탈모는 더 이상 진행되지 않는데 프로페시아가 이런 역할을 한다.
DHT는 남자는 앞머리와 정수리 머리카락이 점점 연모화되고, 여자는 정수리 가르마를 중심으로 점점 연모화되는 안드로겐형 탈모를 유발한다. 안드로겐형 탈모는 전체 탈모 80%를 차지할 정도로 가장 많은 탈모 형태다.
탈모인들에게 한줄기 단비 같은 존재인 프로페시아, 그러나 모든 남자들에게 머리털을 돌려주는 행복감만 선물하지는 않았다. 그것은 바로 부작용으로 성욕감퇴, 발기부전, 사정량 감소등이 발생할 수 있기 때문이다.
남자의 자존심 정력~! 대부분의 남성들은 정력감퇴를 치명상(?)으로 생각한다. 물론 이런 부작용이 3% 이내로 적지만 복용을 꺼려하는 요인이 된다.
프로페시아는 또 하나의 단점이 있다. 가임기 여성에게 사용할 수 없다. 그 이유는 복용 중 임신하게 되면 남성태아의 성기 발달에 영향을 미치기 때문이다.
남녀 모두 안드로겐형 탈모치료 핵심은 DHT호르몬을 억제하는 것이다. 따라서 프로페시아 복용은 최선의 선택이다.
하지만 부작용 때문에 약물 복용을 주저하게 된다. 이러한 점을 이용하여 탈모에 좋다는 각종 제품들이나 비법들이 탄생하게 된다. 심한 경우 탈모약 부작용을 나열하면서 자사 제품을 선전하는 일이 비일비재(非一非再)하다. 많은 탈모인들은 여기에 현혹되어 시간과 비용을 낭비할 뿐만 아니라 아까운 머리털만 잃게 된다.
그렇다면 탈모약을 복용하지 않고 DHT호르몬을 억제하여 탈모를 치료를 할 수 있는 방법은 없을까? 바로 2가지 바르는 약이 있다.
남성은 피나스테리드가 주성분인 핀쥬베, 여성은 알파트라디올(alfatradiol)이 주성분인 엘크라넬이 있다. 핀쥬베는 DHT를 생성시키는 5알파 환원효소를 억제하고, 엘크라넬은 5알파-환원효소와 17β-HSD(17β-Hydroxysteroid dehydrogenase)효소 활성도를 저하시켜 DHT를 감소한다. 또한, 아로마타아제(aromatase)의 활성도를 높여 테스토스테론을 에스트라디올로 전환시켜 탈모 진행을 막는다.
탈모약 부작용이 걱정되어 복용을 꺼려하는 탈모인들에게 두가지 약물은 최선의 대안이 될 수 있다.
두가지 약물과 함께 미녹시딜, 트레티노인, 성장인자, 항산화제 등을 병행하여 치료하면 빠르게 탈모를 개선할 수 있다.

Prostate enlargement and hair loss are caused by a substance called dihydrotestosterone (DHT). Therefore, if the production of DHT is suppressed, hair loss does not proceed any further, and Propecia plays this role.
