지난 2023년 3월 배우 유아인씨의 마약 사건이 이슈가 되고 있었다. 당시 뉴스에서는 유아인씨가 프로포폴 투약 뿐 아니라, 마리화나, 코카인, 케타민, 메스암페타민까지 줄줄이 검출되어 파장이 되고 있었다. 흔히 3대 마약이라고 하는 것이 코카인, 헤로인, 필로폰 이 3가지를 말한다. 보통 이 세 가지의 마약이 굉장히 중독성이 강하고 또 환각작용이 굉장히 센 편이다. 코카인은 백색가루라고도 하는데 주로 미국, 남미, 유럽 이런 데서 굉장히 많이 사용하고 흡입 및 투약하는 마약이라 볼 수 있는데 사실 한국에서 그렇게까지 코카인을 투약하는 사람들이 많지도 않고 일반적이지는 않다. 이태원이나 홍대 같은 곳에서 비밀리에 유통되는 것은 대마라 불리는 마리화나와 엑스터시 종류이고 코카인이 들어오는 경우가 별로 없다. 그럼에도 코카인이 요즘 한국에서 발견 빈도 수가 높다는 것은 남미 쪽에서 들어오는 유학생들이나 노동자들의 수효가 많아졌고 이들을 통해서 비밀리에 유통되고 있기 때문이다.

케타민의 경우는 매우 생소한데 이는 전신마취제이며 의료용 마취제의 원료이고 이것이 극성을 띄면 염소 원자로 치환되는 클로로포름(Chloroform)이 된다. 즉, 케타민의 극성으로 가는 과정의 물질이 클로로포름(Chloroform)이기 때문에 매우 유독하며 미숙한 마취의가 처방할 경우 환자가 중태에 빠지거나 심지어 사망하는 의료사고가 잦았을 정도로 심각한 현상을 초래한다. 게다가 케타민이나 클로로포름은 오용이나 남용되면 환각작용이 발생하고 중독성도 굉장히 강하다. 초기 소련군 지휘관이자 정치인이었던 미하일 프룬제가 사망한 원인이 케타민이나 클로로포름의 과다 흡입 때문이었다. 그런 이유로 케타민이나 클로로포름과 같은 마취제 용도는 일반인들에게 유통이 엄격히 금지되어 있다. 코카인의 경우, 코카나무의 잎에서 추출하는 물질로 여러 가지 화학제품 같은 걸 투입하고 해서 거기서 추출을 하는데 정제를 하는 과정에서 마약으로 유통이 된다. 특히 멕시코, 콜롬비아, 베네수엘라 같은 국가에 마약 카르텔이 있는데 이들은 거의 코카인을 취급한다.
이들은 미국에 수출을 하여 미국인들에게서 코카인이 문제가 되었었고 미국에서 워낙 이 부분에 대해서 단속을 하니까 또 유럽으로 건너갔다. 코카인을 하면 정맥주사를 놓거나, 불로 가열해 코킹하고 1달러 지폐를 돌돌 말아 비강흡입을 하는데 흡입을 할 때, 가루가 코에 걸리며 코가 매케해지기 때문에 자주 코를 만지거나 코 끝이 홍조를 띄는 현상을 볼 수 있는데 전형적인 코카인을 흡입한 이후의 현상이라 볼 수 있다. 코카인은 뇌 도파민 활성을 크게 증가시켜 약효가 있는 동안은 쾌감과 집중력, 창의성과 삶의 의욕을 극도로 솟아나게 한다. 하지만 계속 복용하면 수면장애, 인성장애 등의 정신적 장애가 생기고 폭력, 반사회적인 행동의 증가 등을 유발하게 되며 환촉 및 환각, 편집성 망상 등의 정신적 장애로 이어질 수 있다. 코카인의 중독성, 환각성 그리고 대량으로 재배하다 보니까 가격이 많이 낮춰졌다고 한다.
필로폰의 경우, 메스암페타민이라 한다. 메스암페타민은 최근에는 향정신성의약품 중에서 향정신성의약품은 의약으로 쓰이고 있다. 그래서 굉장히 통증이 너무 심한데 이걸 일반적인 약 가지고 어떻게 보면 통증이 제어가 안 될 때 마약 성분이 들어가 있는 케타민, 펜타닐 가지고 진통을 제어하는 것이다. 그보다 더 강력한 것은 모든 마약의 끝판왕이라 말하는 헤로인(Heroin)이다. 헤로인은 양귀비로 만드는 아편을 정제한 다음 가공해서 만드는 마약으로 이를 최초로 제조한 바이엘사가 모든 약 중의 영웅이라는 뜻의 Hero라는 상표명이 지금 호칭의 기원이라 볼 수 있다. 헤로인은 1874년 런던 성 메리 병원의 화학자였던 몰더 라이트와 그의 동료들에 의해 첫 합성 성공 이후 펠릭스 호프만의 재합성으로 상용화되어 나타난 의료 진통제의 용도로 사용되었다. 당시에는 모르핀이 매우 널리 쓰이고 있었지만 점점 그 심대한 해악이 드러나면서 세계 유수의 제약 회사들에서 보다 안전하고 중독성이 없는 모르핀을 대체할 수 있는 약물을 합성하고자 했다.
바이엘은 이 신약을 '모르핀보다 안전하고 중독성이 없는 약'으로 홍보하고 처방전 없이 누구나 살 수 있는 일반의약품으로 판매했다. 헤로인이 모르핀보다 훨씬 중독성이 강하고 부작용이 심각하다는 사실이 밝혀지기 전까지, 헤로인은 1910년대까지 수많은 약국에서 어린이도 먹을 수 있는 안전한 진해제로 팔렸다고 한다. 그러나 심각한 부작용이 발생하고 강한 중독성으로 인해 여태 강한 마약이라는 아편의 수치를 뛰어넘게 되니 비로소 심각함을 인지한 미국 연방 정부는 결국 1914년 규제 법률을 통과시켜 헤로인은 처방전이 필수인 전문의약품으로 지정되었고 1924년 약전에서도 삭제되고 모든 사용이 금지되었다. 금지되기 전까지 장장 29년간 약국에서 판매되었고, 수많은 중독자와 부작용으로 인한 사망자들 또한 기하급수적으로 늘었다. 이에 1925년에는 국제연맹에서 헤로인을 금지했고, 3년 후 국제연맹 회원국 전부에 적용되었다. 헤로인은 연기를 마시거나 먹는 다른 마약과 달리 비강 흡입, 경구, 주사, 흡연 모든 방식의 투여가 가능하나, 대부분의 중독자가 결국에는 가장 강렬한 주사 투입까지 가게 된다.
이에 헤로인 성분은 피에 섞여 혈관을 통해 뇌로 침투해야 약효가 시작되는데, 다른 방식은 혈관으로 흡수되는 과정에서 다른 장기를 거치고, 그 과정에서 약효에 손실이 발생한다. 그러나 주사는 혈관으로 직접 들어가기 때문에 약효에 손실이 거의 없다. 헤로인을 비롯한 마약 가격이 비싼 이유가 이 때문인데, 다른 투여 방식으로 주사 투여와 동일한 효과를 얻으려면 더 많은 양이 필요하기 때문에 그렇다. 헤로인의 주사 투여 방법은 헤로인 가루를 숟가락 위에 올려 물에 갠 뒤에 불에 가열해서 녹여 주사제를 만들고 주사하는 것이다. 헤로인은 알칼로이드 성분을 갖고 있기 때문에 그 상태로는 물에 거의 녹지 않지만 의약용으로 판매되는 헤로인은 다른 알칼로이드 약물들과 마찬가지로 분자에 산 이온을 붙여 염으로 만들어 물에 녹기 쉽게 만들어져 있다. 당연히 마약 투여인들은 수많은 주사바늘 자국이 팔에 남게 되고, 중독자들이 헤로인에 섞어 넣는 산성 물질의 양 조절을 제대로 할 수 없기 때문에 산에 의해 서서히 혈관이 망가지게 된다. 말기까지 가면 팔에는 주사를 놓을 데가 없어서 다리 쪽 혈관, 그 다음에는 목, 유방, 손발가락의 혈관, 국부에 놓기도 하는 등, 최악의 폐인 모습을 그대로 보여주게 된다.

헤로인이 극악인 이유는 사용을 중지할 때 나타나는 금단현상이 왠만한 다른 마약보다 극악이기 때문이다. 불안 · 불면 · 고민 · 침울 또는 발양(發揚) 등의 정신증세와 함께 구토 · 발한 · 발열 · 설사 등이 나타나고 식욕 부진 · 맥박이 급박해지며 정신지둔(精神遲鈍)이 되어 심신이 함께 쇠약해져 치료가 거의 불가능할 상태로 몰고 간다. 헤로인은 마약 중에서도 정신적, 육체적인 의존 증상과 중독이 가장 극악하다. 인생이 망가지는 것은 물론, 과용 시 호흡 부전으로 사망할 수 있다. 헤로인은 서서히 양을 줄여가면서 끊는 것도 거의 불가능하다. 내성이 강한 헤로인 특성상 반응이 오는 양보다 조금이라도 적게 하면 저 끔찍한 금단증상을 그대로 겪기 때문이다. 그러니까 약물중독을 치료하는 과정에서 죽는 경우가 많아서 살기 위해 약을 해야 하는 상황이 오게 되는 것이다. 그야말로 한 번이라도 하면 인생은 끝이라고 봐야 한다. 따라서 호기심에라도 절대 손 대면 안 되는 약물로 이 약물을 하고서는 온전히 살아남은 경우가 많지않다. 유아인씨가 각종 마약을 가까이한 했다면 국내에서도 최악의 마약인 헤로인도 만만치 않게 퍼져 있다는 것이니 경각심을 가지고 발본색원(拔本塞源)을 해야 한다. 대한민국은 더 이상 마약의 청정국이 아니다. lukybaby7@gmail.com
