‘격화소양(隔靴搔痒)’. 신발을 신은 채 가려운 곳을 긁는다는 뜻으로 애를 쓰지만 정곡을 찌르지 못해 헛수고만 하는 상황을 가리키는 말이다.
필자의 병원을 방문한 고객 중에도 열심히 치료했는데 효과가 없다며 하소연 하는 분들이 있는데, 본인의 탈모원인을 잘못 알고 엉뚱한 약물을 사용한 경우가 간혹 있다. 탈모는 원인에 맞는 검증된 약물로 꾸준히 치료하는 것이 정답이다.
탈모는 발생원인에 따라 다음과 같이 구분이 가능하다.
첫째, 안드로겐형 탈모
안드로겐형 탈모는 유전적 원인에 의한 발생비율이 70~80%를 차지하며 일부는 환경적 원인에 의해 발생하기도 한다. 진행양상은 남성의 경우 앞머리 또는 정수리 모발이 서서히 가늘어지며 이마 M자탈모와 정수리탈모가 진행된다. 옆머리나 뒷머리는 DHT에 대한 민감도가 낮아 탈모가 진행되지 않는다. 여성의 경우 앞머리 부위에는 탈모가 잘 발생하지 않고 정수리 가르마를 중심으로 크리스마스트리 형태로 시작되어 점차 그 범위가 확산된다. 앞머리 부위의 모발들이 잘 유지되는 이유는 테스토스테론을 에스트라디올로 전환시키는 아로마타아제 효소가 많아 DHT가 적게 생성되기 때문이다.
둘째, 휴지기 탈모
스트레스, 다이어트, 영양과잉, 약물, 환경오염, 음주 및 흡연, 잘못된 생활습관, 계절 등 유전과 무관하게 환경적 요인에 의해 발생하는 탈모의 대부분은 휴지기 탈모이다. 굵은 모발이 힘없이 빠지며 심한 경우 하루 200여개가 빠지기도 한다. 특정 부위의 모발만 빠지는 것이 아니고 두피 전체적으로 진행되는 특징이 있다
셋째, 원형탈모
외부 병원균이나 세균 등으로 부터 우리 몸을 보호하는 면역세포가 모발을 해로운 이물질로 인식해 공격해서 생기는 탈모를 원형탈모라고 한다. 면역세포가 어떤 이유로 인해 모발세포를 공격하는지는 아직 명확하게 밝혀지지는 않았으며 초기 원형탈모는 자연회복 되거나 치료를 통해 비교적 손쉽게 치료가 가능하다. 하지만 2~3%정도는 증상이 악화되어 두피 전체적으로 진행되는 전두탈모로 이어질 수 있어 초기에 빠른 대처가 필요하다.
넷째. 성장기 탈모
주로 항암제로 인해 발생한다. 항암제는 암세포가 분열하는 것을 억제하지만 암세포에만 작용하는 것이 아니라 세포분열이 활발한 모발세포에도 영향을 미쳐 모발을 탈락시킨다. 다행히 항암제가 모발을 자라게 하는 줄기세포를 영구적으로 손상시키는 것이 아니므로 항암치료 후에 일정시간이 지나면 새로운 모발이 다시 자라게 된다. 만약 항암치료 후 1년이 지나도 모발이 정상회복 되지 않을 경우 적극적인 치료가 필요하다.
다섯째, 노화에 의한 탈모
나이가 들면서 모발세포가 노화가 진행되어 발생하는 탈모이다. 노화에 의한 탈모는 자연적인 것으로 받아들여 치료를 하지 않는 경우가 많다. 하지만 노화로 인한 탈모는 대부분 모낭의 영양공급이 원활하게 이루어 지지 않아 발생하므로 모낭으로 가는 영양공급을 원활하게 해주면 치료가 잘 된다.
여섯째, 두피질환에 의한 탈모
지루성두피염이나 모낭염에 의한 탈모는 피지가 너무 과다하게 분비될 때 문제가 되며 말라세지아 또는 피티로스포룸오발레균이 번식하면서 피지를 먹고 성장하면서 염증, 홍반, 각질, 비듬을 생성하여 모공을 막아 탈모를 일으키게 된다. 심할 경우 모낭이 손상되어 영구탈모로 진행될 수 있다.
