지방흡입술하고 빼낸 지방, 이를 처리하기 위해서는 의료 폐기물로 모아 놓았다가 폐기물 업체에 줘야한다. 아마도 지방흡입술을 하는 의사라면 누구나 귀찮을 것이다.
버려야 할 지방, 이것을 이용할 수 있는 방법이 없을까? 이를 해결하기 위해서 탄생한 것이 지방이식술이다.
지방이식술이란 지방이 부족한 곳이나 조직 결손부위로 옮겨주는 수술이다. 채취한 지방은 원심분리기 등을 이용하여 순수 지방만을 정제 및 분리한 뒤 주사기에 담아 이식할 곳에 주입한다. 자가지방은 자기 조직이기 때문에 안전하고 이물감이 없고 자연스런 효과를 얻을 수 있으며 시술이 간편하고 흉터가 없는 장점이 있다.
25년전 필자가 우리나라 초기에 지방이식을 할 때 의사들 사이에 말이 많았다. 이식한 지방은 시간이 지나면 흡수가 된다는 인식 때문이었다. 하지만 지금은 성형수술의 한 분야가 되었다.
지금도 지방이식은 시간이 지나면 흡수가 된다는 인식이 일반인들에게 퍼져있는데 이는 잘못된 생각이다. 시술자에 따라 효과의 차이는 있을 수 있으나 이식된 지방은 생착된다는 것이 정설이다.
그 예로 지방이식 후에 흡수되지 않고 오히려 주입했던 지방이 커져 제거 수술을 한 경우도 있다. 지방이식을 하고 나면 일부는 분해되거나 흡수된다. 따라서 채워주고자 하는 지방량의 130%를 주입해야 시간이 흘러도 원하는 만큼 채워지게 된다.
필자는 1년에 지방이식을 700~800건 이상 시술을 했다. 하루 많게는 8건 수술한 적도 있다. 이 때문에 지방이식 후의 결과에 대해 누구보다 더 잘 알고 있다.
최근에는 처리 과정이나 시술방법 발달로 인해 단점이 보완되어 점점 생착률을 높여가고 있다. 또한 줄기세포나 자가 혈청 물질을 이용하여 이식한 지방 생착률을 높이려는 시도가 계속되고 있다.
천덕꾸러기로 취급받았던 지방, 피하조직(코, 입술, 턱, 이마, 볼 등)에 주입하여 도톰하게 하는 필러(filler)로 유용하게 사용되어 또 다른 아름다움을 얻을 수 있다.
게다가 최근에는 줄기세포 보물 창고로 많은 스포트라이트를 받고 있다.
