원형탈모 치료에 있어서 탈모부위에 스테로이드 주사 투여는 가장 효과가 입증된 방법이며 의료보험이 적용되어 치료비용이 1~2만원으로 저렴하다. 하지만 부작용에 대해 너무 과대하게 부풀려져 있다.
인터넷에서 돌아다니는 스테로이드 주사의 대표적인 부작용으로 첫째, 두피의 두께를 얇게 하여 함몰이 생긴다. 둘째, 부신을 억제하여 각종 질병을 일으킨다. 셋째, 면역력을 떨어뜨려 원형탈모 재발률을 높인다. 등 여러 가지가 있다.

스테로이드의 가장 많은 부작용이 주사 부위의 두피함몰이다. 두피함몰이 생기는 이유는 스테로이드 용량이 많거나 주사 횟수가 많아서 피부 진피가 위축되어서 생긴다. 따라서 적절한 용량과 횟수를 지킨다면 두피함몰은 생기지 않는다. 설령 두피함몰이 발생했어도 6개월이 지나면 회복되기 시작하기 때문에 걱정할 필요는 없다.
그러나 간혹 주사 후에 두피함몰이 생기는데 이때는 스테로이드를 더 이상 주사해서는 안 된다. 자칫 진피층을 소멸시켜 모낭이 소실되어 일명 '땜빵'이라 불리는 영구탈모로 진행될 수 있기 때문이다.
또한 원형탈모의 발생과 함께 두피함몰이 시작된 사람이 제법 있다. 이런 경우는 스테로이드 주사를 하게 되면 진피의 위축을 가속화하여 두피함몰이 심해지게 된다. 이때는 바르는 스테로이드인 플루오시노니드(fluocinoid) 연고를 사용하는 것이 좋다.
간혹 스테로이드를 주사하지 않고 PDRN(polydeoxyribonucleotide)을 주사하는데 두피함몰을 방지하는데 도움이 된다.
일반적으로 원형탈모는 치료 전 특별한 검사가 필요 없지만 두피 여러 곳으로 확산되는 다발성이거나 탈모부위 크기가 큰 경우 다른 자가면역질환 동반여부에 대한 검사가 필요하다. 또한 이런 경우는 스테로이드 주사보다는 바로 사이클로스포린 같은 면역억제제를 복용하는 것이 효과적이다.
