피부미용에 관심 있는 분이라면 한번쯤 들어봤을 PRP주사 치료. 피부의 흉터 개선 및 피부 색소침착, 피부톤 개선, 피부탄력 회복 등의 미용 목적으로 피부과에서 시술하고 있는 주사요법이다. 탈모치료를 비롯하여 테니스엘보우나 골프엘보우, 관절통증 완화 및 관절 기능개선의 목적으로 사용하기도 하며 이와 관련 2019년 11월 건강보험심사평가원은 PRP를 이용한 상과염(팔꿈치 관절 염증) 치료법에 대해 유효성을 인정했다.
PRP란 platelet-rich plasma의 줄임말로 ‘혈소판(platelet)이 풍부한 혈장(plasma)’이란 뜻이다. 흔히 ‘자가혈주사’라고 부르기도 하지만 ‘자가혈소판풍부혈장’이 보다 정확한 용어이다.
혈장(plasma)이란 혈액 속의 유형성분인 적혈구, 백혈구, 혈소판 등을 제외한 액체성분을 말하며 담황색을 띠는 중성의 액체이다. 혈소판(platelet)은 지혈이나 혈액의 응고, 새로운 결합조직(connective tissue)의 재건 및 혈관재형성(revascularization)등에 관여한다.
PRP주사치료의 과정은 다음과 같다.
자신의 혈액을 채취하여 원심분리를 하면 혈액이 3개 층으로 분리된다. 제일 윗 부분에는 혈소판, 적혈구 등 유형성분이 존재하지 않는 순수한 혈장, 가운데 부분에는 혈소판이 풍부한 혈장, 제일 아래에는 적혈구가 위치하게 된다. 이렇게 구분된 혈액에서 가운데에 위치한 혈소판이 풍부한 혈장을 피부나 두피 또는 관절 등 원하는 부위에 주사하는 것으로 시술이 끝난다.
혈소판이 풍부한 혈장을 사용하는 이유는 혈소판에는 다양한 종류의 성장인자(growth factor)들이 포함되어 있기 때문이다. 성장인자란 세포간 신호전달에 사용되는 단백질이다. 활성산소, 세균 및 바이러스 등 다양한 원인에 의해 세포가 사멸되거나 그 기능이 저하되면 이웃한 세포에 신호를 보내 세포분열을 통해 필요한 세포수를 유지하게 한다. 즉, 성장인자는 세포의 분열 및 증식에 핵심적인 역할을 한다.
성장인자는 모발의 성장에도 필수적이다. 탈모치료시 원인에 맞는 약물의 사용과 함께 성장인자를 이용한 치료를 병행하면 모근세포분열이 촉진되어 모발이 자라는 속도를 향상시킬 수 있다.
모발성장에 도움을 주는 대표적인 성장인자에는 IGF-1(Insulin-like Growth factor), VEGF(Vascular Endothelial Growth factor), bFGF(basic Fibroblast Growth factor), PDGF(Platelet derived Growth factor), SCF(Stem Cell factor), KGF(Keratinocyte Growth factor) 등이 있다.
PRP주사의 장점은 자신의 혈액을 이용하므로 알레르기 반응이나 부작용이 없고, 간단한 주사요법이므로 별도의 회복기간이 필요치 않으며 짧은 시술시간 등을 들 수 있다. 하지만 치료시마다 매번 채혈을 해야 하는 번거로움이 있고 PRP를 통해서 얻을 수 있는 성장인자의 양이 제한적인 단점이 있다.
이러한 단점은 줄기세포 배양 과정에서 생성되는 성장인자를 이용한 제품을 탈모치료에 사용하면 보완할 수 있으며 보다 간편하고 효과적인 탈모치료가 가능하다.

Advantages and disadvantages of hair loss treatment using PRP injection

It is an injection therapy used by dermatology for cosmetic purposes such as improving skin scars, skin pigmentation, skin tone improvement, and skin elasticity recovery. In addition to hair loss treatment, tennis elbows, golf elbows, joint pain relief and joint functions are also used for the purpose of improving joint function.In this regard, in November 2019, the Health Insurance Review and Assessment Service recognized the effectiveness of the treatment of epicondylitis (elbow joint inflammation) using PRP did.
PRP stands for platelet-rich plasma, which means'platelet-rich plasma.' Often referred to as “autologous blood injection”, “autologous platelet-rich plasma” is a more accurate term.
Plasma is a liquid component excluding red blood cells, white blood cells, and platelets, which are tangible components in the blood, and is a neutral liquid with a pale yellow color. Platelets are involved in hemostasis or coagulation of blood, reconstruction of new connective tissue, and revascularization.
The process of PRP injection treatment is as follows.
When one's own blood is collected and centrifuged, the blood is separated into three layers. At the top, platelets, red blood cells, and other tangible components do not exist, pure plasma, platelet-rich plasma at the center, and red blood cells at the bottom. The procedure ends by injecting plasma rich in platelets located in the center of the separated blood into the desired area, such as the skin, scalp, or joint.
The reason for the use of platelet-rich plasma is that platelets contain various types of growth factors. Growth factors are proteins used for signaling between cells. When a cell is killed or its function is deteriorated due to various causes such as free radicals, bacteria and viruses, it sends a signal to neighboring cells to maintain the required number of cells through cell division. In other words, growth factors play a key role in cell division and proliferation.
Growth factors are also essential for hair growth. In the treatment of hair loss, if the use of a drug suitable for the cause and treatment using growth factors are performed concurrently, the division of hair follicle cells can be promoted and the rate at which the hair grows can be improved.
Representative growth factors that help hair growth include IGF-1 (Insulin-like Growth Factor), VEGF (Vascular Endothelial Growth Factor), bFGF (basic fibroblast growth factor), PDGF (Platelet derived Growth factor), SCF (Stem Cell Factor). ), KGF (Keratinocyte Growth factor), and the like.
The advantage of PRP injection is that it uses its own blood, so there is no allergic reaction or side effects, and because it is a simple injection therapy, it does not require a separate recovery period, and there is a short procedure time. However, there is a drawback of having to collect blood every time during treatment, and the amount of growth factors that can be obtained through PRP is limited.
