보톡스는 주름살을 없애는 명약이다. 우리가 흔히 알고 있는 ‘보톡스’는 미국 엘러간사의 상품명이고, 정확한 명칭은 보툴리눔 독소(botulinum toxin)이다.
보톡스(Botox)라는 말은 라틴어로 검은 소시지를 의미하는 보툴루스(Botulus)에서 유래했다. 예전 유럽에서 썩은 소시지를 먹고 식중독이 걸린 것을 보툴리즘이라고 부른데서 연유한 용어이다. 19세기 초 독일에서 식중독으로 200명 이상 목숨을 잃은 사건이 발생했다. 당시 독일 의사 유스티누스 케르너는 썩었거나 보관이 잘 안된 소시지나 통조림에서 나오는 ‘보툴리눔 독소’가 원인이라는 사실을 밝혀냈다. 이 독이 근육을 마비시켜 사람의 생명을 앗아간 것이다.
이처럼 인체에 치명적인 보톡스는 1973년 미국 안과 의사 앨런 스코트가 원숭이 실험에서 안구를 움직이는 근육이 지나치게 수축된 것을 보톡스로 약화시킬 수 있다는 논문이 발표되면서 질병치료제로 쓰이기 시작했다.
보톡스의 기능은 다음과 같다.
우리가 몸을 움직이려면 대뇌에서 명령을 내려 뇌 →척수→ 운동신경 →근육의 단계로 전달되어 움직인다. 이때 운동신경에서 아세틸콜린(Acetyl choline)이라는 신경전달물질을 근육에 분비하여 몸을 움직이게 한다. 보톡스는 아세틸콜린을 분비하지 못하게 잡아두게 하여 근육을 못 움직이게 만드는 역할을 한다. 신경에서 근육으로 접합되는 부위에서 방출되는 신경전달물질인 아세틸콜린의 분비를 막아 근육수축을 억제하기 때문에 근육의 가역적인 마비가 일어나 근육을 움직이지 못하게 하거나 근육의 힘을 약화시키는 작용을 하는 것이다.
1976년부터는 신경과 의사들이 보툴리눔 독소(botulinum toxin)를 근육의 과도한 수축으로 생기는 뇌성마비나 안면경련, 안검경련 등에 근육을 이완시키는 치료 목적으로 사용되었다. 그러다가 1990년대에 접어들면서 본격적으로 주름살을 치료하는데 이용되어, 지금은 그 수요가 폭발적으로 늘어나는 추세이다. 현재는 사각턱 교정술이나 종아리 축소술 등의 다양한 미용적 목적으로도 널리 사용되고 있으며, 전립선비대증, 질경련, 방광기능장애, 통증치료, 편두통, 다한증 등에도 효과가 있는 것으로 알려져 치료영역을 넓혀 가고 있다.
최근에는 보톡스가 안드로겐형 탈모 치료에도 효과가 있다는 연구 발표가 있었고, 실제 일부 병원에서는 치료에 쓰이고 있다.
보톡스가 탈모치료에 효과가 있는 이유는 다음과 같다.
첫째, 모낭세포에서 탈모를 유발시키는 TGF-β1의 분비를 억제하여 성장기 모발의 기간이 연장되고, 휴지기 모발 탈락이 억제되어 탈모가 감소하여 모발량이 증가한다.
둘째, 두피근육을 이완시켜 모세혈관의 혈류량이 증가되어 모낭에 산소와 영양분이 증가하여 모발 성장에 도움이 된다.
하지만 보톡스 단독으로 안드로겐형 탈모를 치료하는데는 분명 한계가 있다. 그러나 기존의 탈모치료 약물과 병행한다면 효과를 높일 수 있는 또 하나의 무기가 탄생한 셈이다.
